A back adjuster is a chiropractor. lol

 

If you are talking about the back brakes or shoe brakes in general it is the
device foe putting tension on the shoes getting them closer to the drum.

When I did mine I had the same problem.  I bleed all brakes a few times, but

no pressure at the pedal.  It ended up that the back adjuster needed to
turned

a few times to get pressure.
