Personally never experienced it nor heard of one actually blowing-up.
Fittings/plumbing failures yes. Is/was it the type with screw-in end plates
or circlip retained "plug" type? If the latter, perhaps a circlip was
displaced? Is the actual housing distorted? No answer, just more questions!
