Yup.  You have to remove the grommet from the firewall, then remove the
cable from the firewall, then stretch the grommet off over the smaller end
cable nut.  Then you stretch it back on over the smaller end cable nut of
the new cable, put it through the firewall, and then reset the grommet in
the firewall.

Pretty common for the old grommet to be hard and brittle and to break
during this process.  Good idea to have a new grommet in hand before you
start, otherwise you may be doing it all over again later.
