 One and two sound correct.  I'm not sure how a blown head gasket could
cause crankcase overpressure.  The only way I know of (and I'm relatively
ignorant) generating excessive crankcase pressures are, in order of nastiness:
1) Installed unventilated front tappet cover, causing natural blowby to
vent through the cap, dipstick.  Or clogged vent tube/PCV valve.
2) Bad rings.
3) Cracked block.

Needless to say, check #1.  You can hope and pray that the blown head
gasket did not lead to an overheated engine that the DPO attempted to
correct by immediately putting cold water into, leading to #3.
