Scott,

     I use one quite often.  Mine only has a single hole for the stud to go
through, but it's large enough for any stud that I'm liable to run into. I'll
guess that yours is basically the same.  You should see that the disc shaped
piece into which the 1/2" drive extension fits is knurled around its edge.  The
idea is that these knurls bite into the stud and grip it.  The tool can be used
to insert or remove studs by reversing the position of the knurled surface on 
the
stud.  To remove a stud, you will be turning the socket wrench CCW.  Looking 
down
into the hole in the tool that the stud goes through, the knurled edge of the
disc wants to be on the right side of the stud.  This way the torque will cause
the knurl to bite into the stud so as to turn it CCW.

A note of caution, when using the tool on a stud that is going to be reused,
slide the tool far enough down on the stud so that the knurl is below any
threaded section.  If you need more info, let me know and I'll create some 
images
of the tool in use and send them to you.
