I would say that the problem was in the regulator box and that the voltage
control relay had stuck. I find with these regulators when they work they
can go for years without problems, but once they start acting up you can't
trust them again. I have also experienced new ones failing right out of the
box. I don't mess with them if they fail by trying to file the points are
even adjusting the relays. I just replace them. They aren't that expensive
and in the worst case they can set your car on fire if the cut-out relay
sticks closed after shutting off the car. 

I am sure you will get a lot of other opinions and advise on this, but that
is my two cents.
