I don't think that the first on went out so I will try again.

I think that it is a gear reduction device found on the 1 1/2 to 2 ton 
trucks.  Trucks that are 1 ton and under will use the same speedo and won't 
have that thing there.  At least that is what I have found on the AD trucks.
