I keep saying that "one of these days" I'll fill out the information.  Thanks to
you posting the URL (again), I finally did it.  And.... I encourage others to do
so as well.

In a few days (give others time to fill it out) I'm going to print the info out
and keep it in my DayTimer; my one true companion (am I sick or what???)
