Maybe someone didn't know what they were doing?
The head was not checked for flatness? Head not torqued correctly?
Poor quality head gasket? 
I had a 1500 1969 Datsun head job not go right. The next time around I used
Copper Coat Gasket Compound (spray can) on both sides of the head gasket and
torqued the head bolts an extra 10 ft-pounds over spec. I needed this
vehicle and just did what I had to do. ..bill

Also on my rounds today was a stop to lend a hand with a young fellows 66'
Sprite.  It was thought that he had a carb problem but by the time I arrived
it had been determined that he currently had a 2 cylinder engine. So off
came the head to discover that the head gasket was blown out between numbers
1 and 2 cylinders.  Now comes the real question, the engine was recently
rebuilt and only has a 150 mile, what would cause a blow out of almost on
inch of head gasket?  I know it has been discussed in recent times about
what is the best head gasket but my being mobile sort of limits what and how
much data I can keep or access.  It would be nice if I could make a good
suggestion before they just order one from the usual source.
