OT - Vehicle Lien & Bill of Sale?

My dad is selling his "non-roadster" vehicle (Lincoln). He wants to make a
"vehicle lien" and "bill of sale" for the new owner, but isn't sure what he
needs to include in them. I have posted what he has so far to the following
