i used a door from Hagan. i got a 90 degree flat one and welded it into the 
drivers side rear fender on the back side (facing rearward) i dont have it 
finished (ie, no filler) but its welded in and nice. took about 2 hours to 
cut out the fender, flange it, and weld and grind. i also have it positioned 
so that the door opens against the bed, so theres plenty of room for the 
filler nossle. when i get pics uploaded ill let everyone know.
  the Hagan door is expensive (i paid $115) but its a very clean instal, and 
when done, all you can see is a little square dark line, as it uses no 
finger hole to open, but rather a latch like on enternainment centers, where 
you push it, and it pops out, and out then pull it open.
