Got mine from grumpy's truck parts here in phoenix. It's a reproduction but
I've had no issue with the accuracy. Website is http://grumpytruckparts.com.
Manual is $20. They have reproduction owner's manuals as well.

I think I've figured out
which order the pieces go in the ball sockets but it would be nice to verify.
