By your descriptions, I had guessed it was a 1500.

1. Pilot bushing. Mine just slid right in, and it seems to be held from sliding 
out by the flywheel. I had no problems with my alighnment tool.

2. Starter spacer. I have the spacer, but I haven't fitted up the starter bcak 
yet. Now I think I need to check it out!

3. Not sure it matters

4. My pressure plate bolts were Allen head, I assume this is typical.
