I had this problem with my 59 GMC . The body shop tech cut an access hole through the door skin, fixed the hinge mount , then welded up the hole. Sounds brutal but you can't tell now its in primer an

I think you are refering to the torpedo like extensions to the bumper not a guard that extends up in front of the grill? If so these were stock on 1958 trucks and were removed for the 59 model . Eri
