You can use the generec form, no additional info needed. Just indicate
what you are paying for. The entry fee is $35.00 - Extra dinner tickets
are $10/ea - 90 Trial Memberships $15

The $35 entry fee is for all entries postmarked by Monday, June 7th.
After June 7th there will be a $15 late fee. After Wednesday June 9
there will be a $20 late fee.
