I haven't had this problem, but here's an idea.
Take a Dremel and one (or more) of the cutoff disks. You know what I
mean, they are thin carbide grit disks used for cutting music wire
or who knows what else. Use the disk to grind a slot in the stud.
It will certainly grind right through the screw extractor. Then put
a screwdriver in the slot and back the stud out. Of course, if the
stud is small compared to the radius of the disk, you won't be able
to cut into it deeply enough to do any good without also cutting into
the head. If the stud is well and truly stuck, you won't be able to
get it out with a screwdriver even if you can get a good slot cut.
Perhaps an impact-style screwdriver would help. Penetrating oil can't
hurt either.
