I tried just about all the procedures recommended on this list.  Lots of good
ideas which resulted in some improvement.
Finally, using the old fashioned method of having an assistant pump up the
brakes, open the bleeder, pedal to floor, close the bleeder,  pump again,
open..etc. etc.   repeat for all four corners.  I tapped the MC with a hammer
occasionally during the process.  Finally saw some milky fluid from the left
front (not visible bubbles, just cloudy fluid), continued until fluid ran
clear.  Brakes are fine now.

I think banging on the MC to release trapped air, combined with the old
fashioned pedal bleed did the trick.
