Roger, at the root of the blades where they transition to the hub there are
semi-circular cutouts.  The blades flex and the fatigue cracks start at
those cutouts.  The first thing to notice would be paint flaking off in that
area.
Moss actually replaced my first 6-blade fan that failed with another one
just like it.  I guess they got some other subsequent reports of failures
and sent out a bulletin to remove the fans and return them.  When I removed
the second fan, I saw the chips in the paint exactly where the first blades
had failed.
The history of the fans in my car was as follows.  Numbers in parentheses
are the odometer readings at each time:

3 May 95  (31590)  Installed 6-blade metal fan 
16 Jan 96 (34103)  Failed 2 blades, damaged radiator header tank, knocked
fan guard loose and put a 2" gash in it (retained by radiator shroud).
5 Mar 96  (34105)  Installed replacement 6-blade metal fan
14 Feb 97 (35660)  Removed second fan and returned to Moss.  Noted fatigue
cracks beginning in root of all blades (paint chips).

As you can see, the fatigue cracks for the second fan were starting only
1500 miles after installation.

Both fans were yellow 6-blade fans and I believe they were stamped "Made in
China" (or perhaps it was on the box they came in).

I have a yellow, metal, 6 blade fan fitted to my car; origin and age unknown
but suspect was fitted around 2004; just wondering if there are any tell
tale signs to help identify it as one one the Moss/China culprits ??
