Are you using a small spool of wire?

I had the same kind of problem when I was using a small spool of wire.  I
am now using one of the full size spools with the adapter and have not had
any problems.  I think that the sizing of the small spools vary some and
cause this problem.  Your solution of a spacer sounds like a good one.
