Setting up a Suburban to pull goosenecks or fifth-wheels would be a 
little more work, even, than fixing his tag-along problem.

About the only thought I've got is to cut the rear frame horns off a 
junkyard Sub, cut off the upper and lower flanges to fit inside the 
existing rails and overlap by a good foot, weld or bolt them in place 
VERY well and double the upper flanges at the joint with some flat 
stock, then narrow the hitch to fit between the 
now-half-inch-closer-together rails.

Might want to go talk to a shop that does truck frame repair, just as a 
thought.
