I got a reply from John Macartney, who works for the BMIHT and he
answered the two questions I had. In his reply he also included
a site where not only can you get the address BUT also print off
a form to complete. This will save you the mail time over and
back that I expierenced. Below is an excerpt from John's e-mail.
