Jay, Chris, Phil, etc:

As a general rule, a notary public attests to the validity of a signature
on documents (a "subscribed and sworn" or "verification" statement). 
However, depending on the local laws, a notary may also be able to
perform other notarial acts, such as certifying copies of documents as
being duplicates of an original, or attesting to the accuracy or identity
of information provided.

A good start might be to contact the local government agency, and ask
what they require for documentation, and if they have a require internal
form.

Since I am not lawyer, nor do I play one on TV, you should, naturally,
seek the advice and counsel of a competent attorney of your choice.
