Well, it can be done with the engine in the car.  I just did it.  It
isn't something I'd recommend a clausterphobic attempting, though (much
of the bodywork modification was done from inside the driver's
footwell).  Basically, you have to extend the existing "bulge" into the
driver's footwell forward to the firewall.  If anyone's curious about
how, contact me off list.  I'm writing up the procedure.

If I didn't already have a good Isuzu Trooper starter sitting here, I
wouldn't have bothered.
