Andy,
I got my copy free about 22 years ago when I worked as a tech writer at
TI in Plano, TX.  I have since gotten rid of a lot of junk from those
years, but that is one book that I have held onto.  My copy is a XEROX
copy 8 1/2x11 and is 3 inches thick (single sided).
