Tie Rod End Removal

Could someone walk me through the removal
of the outer tie rod assembly of the steering?
I have loosened the tie rod end nut, removed
the nut securing the tie rod to the steering
lever, but can't get the two to seperate. How
do you get the tie rod end to slide up and out
of the steering lever?  I am going to replace the
boot, and the tie rod end boot, thus needing
to remove the outer tie rod.
