Just to the right of his foot is a needle and thread with many loops, 
which I suggest symbolizes "a stitch in time saves nine." Below his foot 
is a broken egg (trust me, it's an egg--I looked up the original larger 
view of it).    Yes, someone got the thing sticking out of his 
head--meaning, "he's got a screw loose."  I determined that the thing 
coming out of his ear is a worm and thought, with the bird on his head, 
that meant, "the early bird gets the worm," but it could also mean 
"earworm," someone bugged by a musical phrase.


As far as the bare foot, there's only one thing that comes to mind, and 
that's the old nursery rhyme, "My son, John, goes to bed with trousers 
on, one shoe off and one shoe on, my son, John. But, that makes no sense 
in the context of old proverbs and sayings.  Wearing only one shoe is 
deemed in superstitious lore to be unlucky, but, I can't find a saying 
that echoes that.
