Yes – it does look like an older Equalizer Systems keypad…  It also looks like it has been modified with both the small switch AND the rectangular switch partially covering the original ‘manual operation’ label.  That leads to the thought that all the rectangular switches may be replacements…
 
So…. You might want to contact the manufacturer and see if they can help you [maybe with a replacement switch panel?].

It is for the hydraulic jacks on my Gulf Stream Conquest on a 2006  Chevy Kodiak chassis. 
The round rocker switch on the upper left corner in this picture is an on-off added by person unknown. 
I want to get an instruction sheet but have no idea who made the system.
