Well in my case the one time, when I opened the gas cap I heard air rush
and what sounded like the tank popping back into shape. If it was vacuum
or under pressure I don't know. I don't remember if the air went in or
out, it was a while ago.

The only way I can see a defective cap causing a problem is if it won't
let air 
IN and causes avacuum that the fuel pump cannot overcome.

You are right though, ODB-II has a fit. It pressurizes the tank ever so 
slightly and if it notices a leak, it puts on the SES light.
