[Shotimes] new clutch failure

I had a customer who I installed a new clutch for a few mths back. HE was shifting at high rpm ( 8000 with a LPM ) and now he has a problem.

He states to me:

"Let me tell you some of the observations I see now. 
With the engine off and the clutch depressed, I can run through all the gears. 
With the engine on, car stopped/idle and the clutch depressed, every gear is difficult to engage.  
Attempting to force the shifter into gear will result in grinding. 
If I work a combination of different clutch positions and engine speed, I can eventually get the shifter to go into all gears. 
If I attempt to drive the car, again the gears are difficult to engage.  Difficult here means that there is a lot of resistance that hadn't been there before.  I believe the transmission has what are called blockers and it feels that these are not opening to let the gear engage.  If I try to force the gear selection, grind usually occurs. 
Because of 3) when driving, it is a nerveracking experience and you have to drive gingerly which is NOT how I want to drive my car. "


I believe  I know what happened ... but like to get the thoughts of the list as well.
