Yes, it WILL cause the lamp out warning light to come on unless you can make
the resistance of the circuit match the original.  The lamp out module
checks both headlights and brake light/turn signals.

As far as I know they do not come with a fuse, mine did not anyway.  You
would need two fuses for both relays.  Might want to look into self
resetting circuit breakers too.  FWIW the low beams would be the only relay
really needed to be protected and this should be with a self resetting CB.

No it will not cause the bulb out light to come on as this only deals with 
the rear bulbs IIRC.
