Ken,
Also old fuel pumps lose pressure and lean things out before they die. They
throw no codes.
Eventually it starts missing at 2500-3200 rpm cruise, the leanest point.
Mine only did it when half-way warmed up. It can get very bad with almost
undriveable bucking..
Other high NOX possibilities are crossfiring plug fires, oil in plug wells,
vacuum leaks, engine pinging, burning oil, etc.Whether you can "feel" it
lean-out or not has to do with how severe  it is. It will show up in the
emissions first, then power, then gas mileage, then seat of the pants feel.
All this assumes no ATX and no EGR.
