What is required to pull this shifter fork?
and what else should I do while I am at it?

You've got a worn 3rd-4th gear shift fork.
Sunbeam Specialties has been out of replacements (actually an updated Arrow
Range part) for several years, and I haven't heard of anyone else who has
them.
Maybe Grimes, in England?
I'm told that a good machine shop should also be able to build up the worn
area and re-machine it.
