The inserts must have been put in to repair damaged aluminum threads. It is
a common thing to do. I have not had any of mine come back out. The big
question is: Did the threaded insert simply screw out?, or did it tear out?
If it simply screwed out then it is a simple matter to replace the threads.
Any Auto Parts House should be able to supply you with the necessary
insertion tool, and a replacement insert. However if it tore out then it
will much more difficult.
