I've never used one of these UJs, just heard others recommend them.

I'd imagine the spider is hollow, allowing grease to flow through it
from the cap with the grease nipple to the other caps - exactly as
happens when the grease nipple fixes to the centre of the spider in the
first place.

I see what you say about the balance - I'd had the same thoughts.
Replacing the nipple with a screw when you are not greasing should help,
but ideally you should have two opposite caps the same (and the other
two could be plain).
