The fact that the smoke is white (you only say "maybe" bluish-tinged), you have
an overheating problem, and it only shows up at sustained higher RPMs, sure
sounds to me like head crack or maybe head gasket leak.  These heads are prone
to cracking (it took me three heads before I found one without a crack).  You
don't mention if you have a 216 or 235 (or other) and don't mention the year of
your truck.  Not sure if any of this info would matter, but might to someone
else.
