First,

Call your human resources person.  Have her verify that:

        you are still covered (Or were at that time).  If your employer  
"forgets" to move you over to the correct insurance you may be  
screwed.  Your employer will bill you and send off some money to  
someone but you may not be covered.
        that all of your personal info is correct.   ALL! Insist that she  
read it off the screen by field, so *SHE* says "last name" and then  
gives it to you by SPELLING IT!  It is VERY common for your last name  
to be listed as your first for example.

Call the Hospital, have the nice lady read the info she has on the  
computer for your account.  Make sure that they have the same info.   
SPELLED CORRECTLY!

        My guess is that you will find someone has the wrong info in the  
wrong place.  Wrong name, wrong Blue cross addy, wrong birthdate.

        Remember, the INSURANCE COMPANY DOES NOT WANT TO PAY YOUR BILL.  If  
your hospital has your correct addy, SSN, mailing address, birthdate  
but the hospital spells your name SAMM instead of SAM, EVEN THOUGH  
THEY HAVE ALL OTHER INFORMATION CORRECT,  any claims will be REFUSED.


        IF ALL OF THE INFO IS THE SAME AT BOTH PLACES get an attorney.  One  
letter will usually fix it.
