I got my gasket from CAT.  A simple fiber gasket.  It fits
both the filter adapter and the block adapter.  Just a ring
about 1/4 inch wide.  No extra holes.  Worked fine in my
MK IA for years.  The trick is tightening the adapter while
the headers are in place.   I wound up undoing the steering
extension and moving it out of the way.  The a 10 inch
adjustable just fit.
