Your suggestion sounds like a good one; if I knew how to weld and had
the right equipment.  I keep thinking about cutting off the bolt heads,
opening the end plate, fishing out the old nuts, and fabricating a
rectangular plate that would fit snugly inside the frame rail.  It would
have two hexagonal holes that would hold the nuts in place (fun time
with a Dremel), so it would almost be as if the captive nuts were still
in place without having to weld anything onto the bottom of the rail
(well, maybe some JB Weld to put the end plate back).  Probably more
work than it's worth, though.

...Then I would make up a repair plate with new welded nuts that can be
offered up so the new nuts go through the drilled holes.  The new plate
is then welded to the bottom of the chassis rails...
