[oletrucks] Matching Rear to Front

I have a '64 Chevy 1/2 ton LWB that I would like change the rear end to
a Ford 8 or 9 inch rear end and put disc brakes on the front. Getting
the two ends to match wheel patterns is where I'm at right now. I've
heard the Granada or an old Lincoln would work with a Chevy truck front
disc but mostly just talk. 
Any info out there would be appreciated.
