I had a clutch failure directly attributed to a roller release bearing.

I'll discuss the result, the cause, and a very simply fix.

The result was that the roller bearing casing overheated and failed.
It scored and ruined the pressure plate,

The cause was that with the combination of the pressure plate and
throwout fork I was using, the roller bearing  was unable to rotate
freely enough because the bottom of the roller bearing housing
interfered with the throwout fork. The result was that the bearing was
lightly engaged all the time, thus overheating it.

The fix was simple, with an angle grinder (a hand rasp will work) I ground
a generous chamfer on the lower rear of the roller bearing casing. You can
easily tell if you need to do this. After fitting the bearing, try and
rotate it in
the fork. If it binds in the fork before the bearing face is vertical, best
to
grind the chamfer.

Mine failed during the '94 6 Hour relay race. We had to replace it (R&R
engine,
find a carbon throwout bearing etc) in just under 90 minutes to get me out
in time.
Luckily a club member lived 20 minutes away and he had a spare one in his
shed!
