lester the king pin and bushing kits come with all 4 bushings and the 2
king pins to replace that stuff.  but!  in order to get the bushings
"right" after they have been driven into the spindle they need to be reamed
and there is a special 2 step reamer wich align reams the two bushings
together.   a machine shop can hone the bushings and hand fit them but it
does not properly align hone the two bushings.   as for the lower fulcrum,
if it rattles when cleaned out, it's usually already worn to the point of
needing replacement.   what i have found over time is the pin wears into
and in extreme cases through the two end bushings silver soldered into the
controll arm's outer end.  
any time you rebuild a front suspension that you do not know any prior
service history you automatically asume the king pins, bushings and fulcrum
pieces are junk.  if upon disassembly you find anything that is within
proper tolerances it is usually some kind of miracle!   same goes for all
the metallastic rubber bushings.  if they are good that usually means
someone recently replaced them before you got the pleasure of recieving the
job.  evry so often you luck out.
