Welding it up and re-drilling is the standard fix, but it is 
temporary - it will last for years, but it will wear again. You
could try putting in an oilite bushing, but they are not
really designed to take high pressure.

What I've done is fit an adjustable pushrod and matching clevis;
that allows me to adjust out the wear. I got them from Moss Europe.
They are close to, but not exactly the same as, the clutch slave
pushrod and clevis.
