Potentially a lot.  However, if it's that badly off, I think you would
notice the lack of power as well.  Just a few degrees isn't going to make
that much difference.

That said, initial timing is something you should be checking at every
tune-up (6000 miles).  And IMO it's a good time to also check that the
centrifugal advance is at least doing something.  Just hook up a timing
light and check that the timing mark moves smoothly as you slowly rev the
engine up to 3000 rpm or so and let it back down.
