Probably because: 1. release bearing worn down too far 2. clutch plate worn down too far 3. pivot pin and/or bushing for release lever badly worn 4. someone has cut length of slave pushrod 5. clevis

Others have mentioned wear, but when my release bearing broke it required two full pumps of the clutch pedal before it pushed the piston out of the bore, I can't see wear giving anything like this am
