Tom,

The door posties can be easily installed even by a person with fat hands as
long as they are using factory parts.

The first thing to do is to remove the postie head from the threaded shank
by holding the last couple of threads with pliers and turning the head with
an appropriate sized wrench.  Then take the threaded shank with the nut,
lockwasher and flat washer on it and put the end through the hole in the
door from the inside.  Next assemble the plastic washer and the head from
the outside.  Tighten the head and the nut and you're finished.

This method was arrived at after an hour of frustrated attempts to assemble
the nut and washers to the shank from the inside of the door.
