Hi Ned,
Using 2 large blade screwdrivers; insert them into the cavity in the middle
of the piston then pry them against each other so that their tips press on
the internal walls of the cavity. 
This will allow you to grip the piston and pull it out.

I am refurbishing the OD. All the mechanical parts look good and move
smoothly. There is a layer of sludge on the bottom of the case and in the
pistons and accumulator. The distance piece and the heavy spring came out
easily but the accumulator piston assembly is still in the case. How do you
get it out??
