Yes.   Most are a 'passive' system, using engine vacuum, but I believe some 
cars actually had a source of positive 
pressure to push the gasses through the system.

No.  It simply dumps into the air filter instead of onto the road.  Nothing 
'positive' (or active) about it.

I don't believe so.  All cars have some blow-by, which pressurizes the 
crankcase.

I put one in so I could run K&N air filters (couldn't figure out a clean way to 
run the crankcase vent into the filter). 
After installing the PCV, I noticed oil consumption was down, the 'wet spot' 
was smaller and I didn't get as much smoke.
