Most likely overtightening of the locknut.  You might check and see
how much of the thread is lost on the adjusting screw as well.  In any
event, you should replace the rocker arm and screw.  The screw gets a
huge pounding.

Piece of cake.  Should take about half an hour.  Flatten the lock
tabs and gently undo the four bolts securing the rocker towers.  That
is, undo each about one turn at a time, in sequence, until the spring
tension is relieved.  Pull the rocker assembly off.  Remove the clip
on the rear end of the rocker shaft and spool each component off the
shaft until the defective one is removed.  Reverse.

Please note that there are several varieties of rocker arm for this
engine (I think three kinds) so you need to be sure and get the
correct one for that position.  Also make sure the components are
replaced on the rocker arm in the exact order in which they were
removed.

Whe you are done, set your valve clearances.
