Could be a symptom of the general poor running

Check all the usual things like valve clearance, plugs, points, timing
(including centrifugal and vacuum advance), carb set up (and possibly
dragging brakes as well!)

Could be worn valve guides

If it drops out of O/D when the gear lever is moved out of the 3rd/4th plane
then it sounds like the manual control switch or wiring, since the gearbox
override switch seems to be doing its job implying that the O/D itself is OK
