I think that I've figured this one out. As to be expected it was
embarassingly simple. It seems that it has been a while since I
cleaned my K&N air filters. I took them off to clean them (doing a
pretty good job on my bathroom sink in the process) and while they were
off took the car for a test drive. It ran just fine. It seems that with
the filters dirty (clogged), at low throttle and mid revs the engine didn't
pull enough vacuum to pull enough air in through the clogged filters.
At higher RPM or full throttle, it sucked hard enough to get the air
through "that tiny straw".
